GRADES 1-5 REGISTRATION
Pick-up or print out the registration form.
Complete all parts of the registration form – front and back. The registration form must be completed and signed by the student’s primary guardian/parent.
Include copies of the following required documents (See Registration Criteria for more information):
Student Social Security Card
Student birth certificate
Up to date Immunization Record
Parent/Guardian picture ID
Proof of Residence – 2 current (within the past 2 months) utility bills
Completed registration packets can be dropped off at the school board office beginning Monday, June 8th between the hours of 8:00AM – 3:00PM. The Parent/Guardian is responsible for having copies of the required documentation included with the registration packet. Due to social distancing requirements, school board office personnel are not able to make copies at this time.
